Open a new document that is 800x800px

In the Magik Lizzy AO F 2.png
Resize that to your liking and copy and paste that to your blank canvas

In the Papers folder open Magik Lizzy AO P 2.png
Resize that to your liking and copy and paste that below the frame layer
Grab your Rectangle Marquee Tool, draw a rectangle around the frame,
Select >> Invert and Delete on your keyboard and Deselect

Open your tube and select the close up layer
Resize and copy and paste below the frame layer
Move this to the left sightly and apply some drop shadow
Click on the layer below, add a new layer, right click on the above and merge down
Change the layer mode to Luminosity and Opacity to 50%
To get rid of the tube that is outside the frame, press shift, click in the layer, Invert and Delete

Ctrl & Alt & Z to make it to the original size
Select the girl layer, resize and copy and paste that above the close up tube, move to right
Apply the same drop shadow and use the same settings as the close up tube
Grab you Plygonal Tool with the Anti-alias off and make a selection that is below the arm
Cut that and paste above the frame layer
Move that so the two line up and you can't see where you have cut
I find that using the arrow keys help with this
Use your Rectangle Marquee Tool to get rid of the bottom

Open Magik Lizzy AO E 46.png from the Elements folder and resize
Copy and past that as a new layer and position
Check my final tag for the position

Below is the order of the other elements that I've used
Up to you on the size and check my tag for the position
If any of the elements are mirrored the go to Edit >> Transform >> Flip Horizontal
